Parents/guardians are encouraged to assess the health of their child every day before they come to school. Please use the following guidelines to help make the decision when your student is ill. Thank you for helping keep our school community healthy!

Illness Guidelines: 

The Health Team adheres to Colorado State regulations governing schools, in conjunction with the Colorado Department of Education, and the Colorado Department of Public Health and Environment. Decisions on sending students home are solely at the discretion of the Health Team based on medical-related conditions or illness. Generally, if your child has a fever greater than 100.4º F, is vomiting, and/ or has diarrhea resulting from illness, they are to stay home; a parental excuse or doctor’s note will be required to excuse the absence. Contact the Front Office Coordinator of your child’s school if you have any questions regarding attendance.

Annual Health Update:

To help us better care for each student, we ask parents/guardians to submit an Annual Health Update, including the latest vaccination records or exemptions, and Health Care Plans (see below), before the start of each school year. 

The Annual Health Update form is completed during the yearly check-in/ registration process.

Immunizations/ Vaccines:

  • Vaccination records/exemptions can be uploaded as part of the online registration process. Vaccination records/exemptions that are not uploaded at registration can be submitted via: 

    • Faxed using our secure fax number 719-355-1819 

    • Hard copy mailed or hand-delivered to the school (Attention: School Nurse)

Medication at School: 

Medications must be administered in accordance with school policy. There must be a reason to administer the medication when the child is at school or a child care setting. (CRS 22-1-119 and 1 CCR 301-113).

For any student requiring medication during the school day, this printable Authorization Form for the Administration of Medication by School Personnel must be completed before any medications can be administered. This form must accompany EACH medication, INCLUDING over-the-counter medication(s). It must be completed by the parent/guardian AND signed by a health care provider with prescriptive authority.

Atlas Preparatory School may only administer medications at school with the properly completed documentation and the medication in the original, properly labeled container. Non-FDA-approved substances, including herbs, supplements, essential oils, etc., will NOT be administered at school.

Students with Severe Food Allergies or Intolerances: 

If your child has severe food allergies or intolerances, please notify the school nurse immediately. If you need special modifications made for school meals, please reach out to our school lunch program regarding the procedures for accommodating children with special dietary needs. 

Health Plan Information: 

The priority for our Health Team is to provide optimal care for your child and ensure they have the medical support necessary to be safe and successful while at school. The health care plans below contain all the necessary documents to allow your child to receive medication at school and self-carry inhalers and/or Epi-Pens®, if desired.

All medications require an Authorization to Administer Medication Form, with their respective care plan. Each medication requires an authorization form. ALL forms must be completed, filled, and signed by a medical doctor/ provider with prescriptive authority in the state of Colorado. Please be aware: medications AND completed forms must be turned in to a Health Technician or (Lead) School Nurse; NO person outside of the Health Team, including front office staff or teachers, is permitted to accept student medical forms or medications. Medications and forms must be turned in together at the same time; no incomplete packets will be accepted.
